Rheumatology in India like many other subspecialties in medical field is in constant need of rapidly adapting the advances in the field keeping in mind the uniqueness of the Indian scenario and due pragmatism on the face of limited resources. In this context, I would like to draw your attention to the topical review in this issue of the Journal by Durga et al., appraising the combination of conventional synthetic disease-modifying antirheumatic drugs in the management of rheumatoid arthritis vis a vis biologic disease-modifying antirheumatic drugs (pages 104-109). It underscores the need to develop India-specific guidelines for the management of rheumatic diseases which must confirm to and reflect the variable access to several therapeutic modalities across India.
